５．Regarding the handling fee for requesting disclosure and using purpose notification
If you request disclosure of your personal information and notification of the purpose of use, 1,000 yen (excluding tax) will be charged for each request.
Please enclose a postal money order for 1,000 yen (excluding tax) with your submitted documents. The cost of purchasing a fixed amount postal money order and the postage to our company will be borne by the customer.
